Navigate
Navigate the system of care during and after emergency visits and hospitalizations while advocating for resources ( ie housing, shelter, food).
Understand
Understand our patient’s goals, motivations, and readiness for change while exploring the reasons for substance use (mental health, trauma, lack of care access).
Assess
Evaluate and diagnose unhealthy substance use to offer appropriate services that meet patient goals.
Treatment
Offer evidence-based addiction services (ie medication, harm reduction, residential) and education.
Community
Link patients to community healthcare and addiction treatment, including residential and other community-based programs.
Destigmatize
Reduce the stigma and harms of substance use through patient-centered communication while using patient narratives to decrease stigma among providers and staff.
